By indicating 'yes' and submitting this online registration form you agree to the following terms and conditions as relating to the student named therein.
By submitting this form, you declare that the information and
medical details are correct to the best of your knowledge and you understand that StageCoach Theatre Arts (“StageCoach”) reserves the right to restrict admission at its own discretion.
Privacy
All information supplied on this form will be kept strictly confidential and StageCoach will never divulge any personal information to a third party without your consent.
Cancellation
Term-time classes
Before classes start: You may cancel your child’s registration and receive a full refund up until 24 hours before your chosen class start. Cancellation within 24 hours of your chosen class start time will result in the loss of your deposit. (Deposit are refunded in full when you pay by cash/check and minus processing fees when paid by credit card.)
Once class has started: Once accepted by StageCoach you do not have to re-register your child or ward at the end of each session and the registration will remain active and continue indefinitely until we receive notice of cancellation in writing. Therefore, you must give sufficient notice (at least four weeks) of your child or ward’s intention to leave StageCoach at the end of any term. If no notice is given, you agree to pay four weeks fees in lieu of notice. (Term-time registration does not include summer camps.)
Summer Camps
Before classes start: You may cancel your child’s registration and receive a full refund up until seven (7) days prior to the camp start date. Cancellation within seven (7) days of a summer camp start date will result in the loss of your deposit. (Deposit are refunded in full when you pay by cash/check and minus processing fees when paid by credit card.)
Once class has started: If you cancel your child’s registration once a camp has started no refund is available and all fees will be payable in full.
Participation and Medical Emergencies
You grant permission for your child or ward to participate in all StageCoach activities, except for any restrictions indicated under 'Medical History'. You understand that StageCoach does not provide health insurance or medical coverage and that by agreeing to the submission of this form you acknowledges your responsibility for payment of any medical treatment which may be required while your child or ward is participating in classes at a StageCoach school.
You further grant permission for StageCoach or our representatives to procure any and all necessary medical help for your child or ward while they are under the supervision of the StageCoach school and authorize StageCoach or its representatives to permit any competent medical person to take all reasonable measures to treat any injury or sickness that your child or ward may suffer.
Liability Release
By submitting this form you hereby release all members of the StageCoach staff and any other party involved in the organization and administration of StageCoach classes and camps from any liability as a result of any injury sustained in or around a StageCoach school location.
Photo Release
You grant permission for any and all photographs taken at StageCoach schools to be used by StageCoach or its representatives in displays or for advertising in, but not limited to, newspapers, magazines and brochures. You understand that no personal information will be associated with any photographs without your consent and that no compensation is offered. You also agree that all photographs taken by StageCoach are the property of StageCoach.
